FORMS/WAIVERS  In order to purchase a Discover McDermont, or All-Access Day Pass, and to participate in the attractions at McDermont Field House, you must sign a waiver. Minors must have a waiver signed by their parent or legal guardian. A Waiver Pass Card may be obtained for free when you submit your waiver. This Waiver Pass Card is valid for one year and will enable you to enter McDermont Field House without a newly signed waiver. If you forget to bring your Waiver Pass Card, you must have a new waiver signed. dfa Download the General McDermont Field House Waiver by CLICKING HERE for English or CLICKING HERE for Spanish. dfa In order to register a team for a sports league held at McDermont, please fill out the attached TEAM REGISTRATION FORM. dfa In order to register as an Individual for a sports league held at McDermont, please fill out the attached INDIVIDUAL REGISTRATION FORM.
